Responsibilities:
The Nurse Practitioner In-Home Assessment are responsible for performing Health Risk Assessments and occasional transitional care for patients discharged from hospitals to prevent re-admissions.
As a Nurse Practitioner In-Home Assessment you will:
Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit Educate and counsel patient and family based on assessment results, including recommendations Performs follow up visits in transitional care
Qualifications:
Current RN and NP licensure in the state of practice Current CPR certification Must be enrolled in both Medicare & Medicaid Recommended Skills Cpr Certified Nurse Practitioner Health Risk Assessments Medicaid Medicare Nurse Practitioner Apply to this job.
